For both in-person and virtual activities/programs, consent must be obtained from each youth participant's parent or guardian. For that purpose, we are providing a template for your customization and utilization.  Please contact our office if you feel that this template is not appropriate for your program/activity or if you would like approval to use an alternate waiver/consent form.

In general, such activities are conducted by third parties, including General Registered Student Organizations.

Third-party users utilizing University property for youth events are responsible for collecting and storing the waivers for a minimum of five years.
